To include Health Equity Matters in your Will, we suggest the following wording:

“I give [insert your choice of % of estate, specific amount, or description of asset] to Health Equity Matters Ltd (ABN 91 708 310 631) of Level 3, 414 Elizabeth Street, Surry Hills NSW 2010, to be used for its general charitable purposes. A receipt from an authorised officer of Health Equity Matters will be a full discharge to my executors.”
